be available or applicable tomorrow. Patient goals today will change and evolve over time. Because nurses work in a changing environment, they must be familiar with change management and strategies for dealing effectively with change. As a care coordinator, you will need to understand how to manage change.
Change management is ubiquitous and involves ensuring that change is sustainable and implemented smoothly. It is important to understand the broad impact on the organization and stakeholders. The change process is complex, and each change initiative you manage or encounter will have its own unique set of objectives and activities, all of which must be coordinated. The nurse engaging in care coordination can better manage care if they understand the change process. There are many models and tools that help address change. Three common change models are the change curve model, Lewin’s Change Management Model, and Beckhard and Harris’s Change Model.
The change curve model is easily adapted to the health care environment and describes change elements that are commonly seen and represents
